Student Registration

Registration for the 2020/2021 School Year is open!
At this time, we are doing telephone appointments to finalize registration.

Please follow these steps to register:

  1.  Complete the online registration through the link below.
  2.  Collect all your necessary documentation using this checklist.
  3.  Scan or take photos of your documentation.
  4.  Email the documentation (with your student's name, grade and the school they will be attending in the subject line) to dschmidt@derbyps.org.
  5.  Call 203-446-3920 to schedule a telephone conference to review the online registration and the completion of your documents.
     






 



You may need to provide original documents for review at some point.

Registration for Little Raiders University is on a first-come, first-serve basis. Spots are limited.
You need to complete the online registration and have all necessary documentation to complete the process.
Registration will not be considered complete without all required documentation - no exceptions.
Placement is not guaranteed as the number of available openings is subject to change. 
You will be notified in July, by phone, if you received a spot and letters will be sent later in the month. 

There is a 24 to 48 hour processing period from the time all required information/documentation is received and residency is confirmed. You will be contacted by the school with information on your student's starting date.

The following documentation is required to register your child - these requirements have been determined by the Derby Public Schools Board of Education. Please use this checklist to track the documents you need to collect. A parent or legal guardian is required to register student(s). 

 
  • The child's original full-size birth certificate (not wallet size, not a photocopy) for all students born in the United States. Students born outside of the United States need to present a passport/visa or green card;
  • The State Health Assessment form (available for download below under “Things You Will Need For Registration”) and a copy of your child's current immunization record;

      Proof of Residency (All of the below items are required)

         * Current Lease/Rental Agreement or Mortgage statement

         * 2 Utility Bills (Phone, Electric, Gas, Water, Cable, Auto Insurance, Auto Registration, Tax Bill)

         * Parent/Guardian CT License with Derby address on the front (change of address label on the back is                           acceptable.)

         * If you rent or lease your home/apartment you must also provide a notarized Landlord/Property Owner
            Certification of Residency (available below under “
Things You Need For Registration”) 
            
Please Note: We do
call the landlord to verify the certification. We cannot proceed with registration until this                is complete and all documentation is received and verified.

 
  • Legal Custody Documentation if applicable

  • Special Education Records (IEP) or 504Plan if applicable
 
  • Students entering high school should provide their most recent report card or transcripts to facilitate creation of their schedule. Without this information, the student's start date may be delayed until we receive this information from their previous school. 

  
A completed Connecticut Health Assessment Record (completed within the last 12 months) including vaccination records, MUST be submitted no later than August 17, 2020 for the student may start school on the first day. All students must have the appropriate vaccinations to enter school. Those with a valid religious or medical exemption are required to submit the appropriate documentation.

Students will not be allowed to enter a Derby Public School facility without the appropriate health records.
