In response to the latest State and National drug crisis the Derby School District will implement a prevention interview process for all 9th graders. Evidence has shown that 9th grade is a particularly vulnerable time for young people as they develop socially and emotionally.
Students will participate in a questionnaire with trained staff and receive important education and information.
All information will be strictly confidential and have no reflection on student behavior.
If you do NOT wish your child to participate please sign this form and return it to the school.
